In-Home Music Lessons Liability Waiver

PARENTAL CONSENT & LIABILITY WAIVER For In-Home Music Lessons Without an Adult Present This Parental Consent & Liability Waiver (“Waiver”) is entered into by the undersigned parent(s)/legal guardian(s) (“Parent”) of the minor student(s) listed below (“Student”) in favor of Hudson Music Studio, its owners, employees, instructors, contractors, affiliates, and representatives (collectively, the “Studio”). 1. Acknowledgment of Policy Waiver I, the undersigned Parent, acknowledge that Hudson Music Studios’ standard policy requires an adult to be present during all in-home lessons where minors are involved. However, I have requested an exception to this policy and consent to my child receiving in-home music instruction without an adult present. 2. Assumption of Risk & Release of Liability I understand that by allowing a teacher from Hudson Music Studio to enter my home while only my minor child(ren) is present, I am assuming all associated risks, including but not limited to: • Any accidents, injuries, or incidents that may occur during or in connection with the lesson. • Any personal, property, or financial losses incurred. I voluntarily release, discharge, and hold harmless Hudson Music Studio and its instructors from any and all liability, claims, demands, or causes of action, whether known or unknown, arising out of or related to the piano lessons conducted in my home without an adult present. 3. Indemnification I agree to indemnify and defend Hudson Music Studio and its instructors against any claims, damages, or legal actions brought by or on behalf of the Student or any third party related to in-home lessons conducted without an adult present. 4. Authorization for Emergency Situations In the event of an emergency during a lesson, I authorize the instructor to take reasonable steps to ensure my child’s safety, including contacting emergency services if necessary. I understand that I am responsible for any costs incurred as a result. 5. Acknowledgment of Voluntary Agreement I acknowledge that I have read this Waiver in its entirety, understand its contents, and sign it voluntarily. I further affirm that I have had the opportunity to ask questions regarding this Waiver before signing.
